New Patient Forms

***As of May 2024, we have a waiting list for new patients (FemilLift patients can be seen immediately).  If you are interested in pellet therapy or weight loss, feel free to send in your forms and we will notify you when an appointment becomes available.***

We primarily see patients for bio-identical hormone replacement therapy (BHRT). To be a patient, you must be done having children and have a reliable or permanent method of contraception (e.g. IUD, tubal ligation, vasectomy). 

Please fill out the "New Patient Packet" below and return it to us if you would like to become a patient. Information on where to send your completed packet is at the top of the New Patient Packet. 

Our Welcome Letter and Notice of Privacy Practices are also below for your reference.
